    Extraction of second molars: a viable option for (non)orthodontic patients

    The debate between extraction and non-extraction treatment in orthodontics remains significant, particularly concerning which teeth to extract when necessary. Traditionally, premolars are frequently chosen for extraction to manage crowding, achieve proper alignment, and improve occlusal relationships. However, second molar extraction has emerged as an alternative with several potential advantages: removing second molars creates space, often allowing third molars to erupt into good occlusion, potentially avoiding future wisdom tooth extractions. Second molar extraction may simplify mechanics, reduce anchorage demands, and shorten overall treatment time. Unlike premolar extraction, it avoids excessive retraction of anterior teeth, preserving a natural facial profile and smile. Timing of second molar extraction varies. Early removal during late mixed dentition can maximize space for third molars, while some suggest waiting until second molar roots are fully developed but before third molars begin erupting. Despite its advantages, second molar extraction has limitations. Not all third molars will align ideally, and some may still require extraction. Factors like jaw size and growth potential must guide the decision. Second molar extraction offers potential benefits but should be evaluated individually.

    Vaska Vandevska-Radunovic received her DMD and MSc in orthodontics at the University “Kiril i Metodij” in Skopje. She continued her orthodontic training at the University of Bergen, Norway 1990-1992, where she obtained her PhD (doctor odont) in 1998. She is currently Professor and Head of the Department of Orthodontics, University of Oslo, Norway and is involved in the education and research activity of graduate, post-graduate and doctoral students. For six years she was a member of the Continuing education committee of the Norwegian Association of Orthodontists. She is Past President of the European Orthodontic Society, 2023, and President Elect of the NEBEOP Board. She is Associate Editor of Clinical and Experimental Dental Research and The Korean Journal of Orthodontics. She is also member of the Editorial board of the World Journal of Orthodontics and Acta Odontologica Scandinavica. Her main areas of interest are tissue reactions during orthodontic tooth movement and relapse, retention procedures and long-term outcome of orthodontic treatment and the effect of orthodontic treatment on periodontal health.
